HMIS Forms
To ensure privacy and consistency in documentation, the LA/OC HMIS Collaborative had developed a number of HMIS forms. Each participating organization must sign a participating organization form along with a user agreement for all HMIS users. Participating organizations must also post a Privacy Notice and provide a copy of the LA/OC HMIS Privacy Policy upon request.
Additional forms include Client Consent Forms which must be signed by each client entered into HMIS and HMIS Assessment Forms (including, intake, exit, and annual assessments).
Participating Organization Forms
Each participating organization must sign the HMIS Participating Organization Agreement and all users of that organization must sign the HMIS User Agreement before being granted access to the LA/OC HMIS.
Privacy Notices
Each participating organization must post a Privacy Notice that explains the uses and disclosures of information. In addition, participating organizations must provide a copy of the Privacy Policy upon request.
Client Consent Forms
Participating Organization must obtain informed, signed consent prior to entering or accessing any client protected personal information (PPI) into the LA/OC HMIS using the Consent to Share PPI Form. Clients that provide permission to enter personal information allow the agency to share client and household data with participating organizations in the region through the Interagency Data Sharing Consent Form. Consent forms must be stored in client records for auditing purposes.
Services will not be denied if a client chooses not to include personal information. If a client refuses to provide consent, the User should not include any personal identifiers (such as first name, last name, social security number, date of birth, etc.) in the client record; Instead, User should include a client identifier to recognize the record in the system.
